Symptoms:
This 2003 Hyundai Sonata check engine light 401 was reported to me the other day by a customer. He had the same experience six months ago when he got the same code and was repaired by replacing the egr valve. Lately, he noticed the check engine light is back together with the vehicle’s symptom of bad gas mileage. When I scanned for codes, I got P0401 code which means “exhaust gas recirculation “A” flow insufficient detected”. This is mind boggling since the egr valve was just replaced.

Searching the database, these are some of the possible fixes:
1.) Clean the map sensor vacuum line for restrictions. Shown here is a picture of the map sensor location on 2003 Sonata. I pointed a red arrow on the map sensor if anyone wants to service it. You can remove it and check the vacuum hose for supply and blockage including the cleaning of the passages with a brake cleaner and air hose.
2.) Service the egr purge solenoid vacuum connections.
The solenoid is a normally open type that closes to supply vacuum to the EGR. If you pinch the yellow vacuum hose at the throttle body and with engine idling, it should run rough or even stall which indicates that a vacuum leak is not present and that the EGR valve is functioning. If it does not open the EGR valve, then a vacuum leak should be checked or verify the presence of vacuum supply problem. The green hose beside the yellow hose in the throttle plate is vacuum supply source. To bench test this egr solenoid valve, use a hand held vacuum pump. When the voltage is applied to the solenoid winding, it should not vent, and the same if reversed.
3.) If both above fixes are tried and still the code remains, make sure the EGR valve used came from the dealer. Some after market egr valve are notorious for failing prematurely.

Having a good vacuum diagram will help you trace the egr system components including the colors of the vacuum hoses and location of the egr parts. ATS provides the diagram if you need it.
